In the event of an emegency, we may be asked by paramedics to provide a list of medications your child takes. If you give us permission to pass this information on, please provide a list below.

Emergency Contact Information

It is a condition of a child's participation in any RBC program that we have the details of two people to contact in an emegency. For the sake of this form, we call these people primary and secondary carers. However, they can be parents, guardians, relatives, trusted friends, etc.
 
 
 
 
 
Conditions of Participation

Behaviour - All children, youth and leaders deserve to feel safe and respected. As such, any behavoiour deemed to be unsafe or unacceptable by the RBC team may result in you being required to pickup of your child from the program.

Personal Electronic Devices - We discourage children and youth from bringing personal electronic devices to RBC programs. Those that do are required to hand them over at sign-in to be stored in a locked room for the duration of the program. These will be returned at the end of the night upon being signed out by a parent/carer. Nevertheless, RBC will not be held liable for the damage or loss of personal electronic devices brought to RBC programs.

Drugs and Alcohol - All RBC programs are drug and alcohol free. If your child is suspected of being under the influence of or in the possession of drugs or alcohol, you will contacted and required to immediately collect your child. The matter may also be referred to the police or other authorities.

Photos and Media - RBC staff/volunteers are not permitted to take photos or videos of participants of RBC programs in unofficial capacities. If photos are required for in-program or promotional use, we will indentify a leader as the official photographer for the event. All captured media will be stored on the official church hard drive only and will never be retained by individual leaders. You will be contacted for permission prior to an image of your child being used for print or online promotional material.

Transportation Policy - In the event that a RBC program occurs away from Church grounds it will be the responsibility of parents/carers to provide transport. In some cases, RBC may seek event specific permission for your child to ride on a hired bus with a professional driver or ride in a staff or volunteers car with two unrelated adults who posess valid Working with Children Blue Cards.

Health Policy - Parents/carers are not send children to any program where there is reasonable suspision of their child being ill.
